Job Description

Overview Physical Therapist•Skilled Nursing Facility travel assignment in Westbrook, Maine. Start date June 22, 2026 for a 13-week contract. Weekly pay range $2,235 to $2,341 with guaranteed hours of 35 per week. Shift is 8:00 a.m. to 4:30 p.m. on a 5x8 schedule. This assignment takes place in a skilled nursing facility setting and is classified as travel•temporary.

Setting:
Skilled nursing facility; travel assignment Responsibilities Conduct initial and ongoing physical therapy assessments for residents to identify treatment needs and goals. Develop, implement, and adjust individualized therapy plans focusing on mobility, gait, balance, strength, range of motion, and functional outcomes. Deliver evidence-based PT interventions and monitor resident progress, adjusting plans as needed. Document evaluations, treatment sessions, clinical findings, and progress notes in the facility's electronic health record; maintain accurate and timely records. Collaborate with the interdisciplinary team (nursing, physicians, case management, therapy assistants) to coordinate care, discharge planning, and transition to home or community settings. Educate residents and families on prognosis, home exercise programs, fall prevention, and safety strategies; ensure patient safety and adherence to care plans.
