Job Description
General Summary:
The Physical Therapist is responsible for, under general supervision of the Physical Therapy Site Managers, developing and administering physical therapy evaluation, treatment, and education programs for the purpose of preventing, maintaining, correcting, and alleviating a physical impairment. May perform these duties at any of the 3 SIRM locations. Currently hiring for the Commerce location.Principle Duties and Responsibilities:
Accepts responsibility for patients assigned and carries out physical therapy program according to standards set by department and APTA. Gathers medical history from the patient records, physicians and/or staff. Tests muscle, nerve, joint and functional abilities as necessary. Accurately and holistically evaluates patients' functional ability related to patient's diagnosis. Able to interpret the results of the evaluation to determine deficits. Continually re-evaluates the patient with formal re-evaluations per APTA guidelines. Establishes realistic goals and designs appropriate treatment programs based on the evaluation findings and patient's goals. Makes appropriate referrals to the interdisciplinary team and through the continuum of care. Selects and administers appropriate patient therapy programs according to APTA guidelines including, but not limited to, active and passive exercises, muscle re-education, gait, functional training, cardiac and pulmonary rehabilitation and the application of appropriate modalities such as cold, heat, light, traction, water, and electrical current. Instructs patients and their families in appropriate home therapy programs including exercise, use of therapy devices and aids. Provides written instructions/guidelines and equipment recommendations as needed. Interacts responsibly with patient care team including physicians, etc. Delegates responsibilities when appropriate. Records and reports patient's evaluations, goals, treatment programs and progress relating pertinent information clearly and concisely in a timely manner using department's current documentation procedures. Completes daily patient billing forms. Ensures accuracy of individual schedule. Assists with program growth and development activities. Participates in development and implementation of new programs, customer satisfaction teams and initiatives, marketing activities and community education. Maintains an acceptable workload, based on department standards, and utilizes time effectively. Actively assists in providing patient care within other SIRM locations when coverage is needed. Maintains a safe working environment. Ensures the collection of data as needed to comply withMIPS/QPP
standards for therapy. Attends staff meetings, in-services and conferences in accordance with department procedure. Maintains current knowledge related to practice excellence in specialty area. Demonstrates and actively promotes an understanding and commitment to the Employee Handbook including Compliance, Commitment to My Co-Workers, Standards of Conduct and Code of Ethics. Performs other related duties as assigned or requested.Vocational Training and Competencies Required:
Master's or DPT degree in physical therapy or an equivalent combination of Bachelor's level education and experience. Must possess current licensure as a Physical Therapist in the State of Michigan. Must possess and maintain current BLS certification. 4.Cognitive and Affective Competencies:
Problem-solving skills including cost-benefit analysis Ability to utilize verbal and written communication skills in order to address and interact effectively and courteously with external and internal customers, including senior management and various other individuals Operate system-based software applications, hospital preferred Analyze, evaluate, integrate, compile and compare data and information Active listening, conflict management and team building This position requires the ability to identify age related differences within the persons served and the adjustment of treatment style and expectations as appropriate, including adjustments based on the person's served cognitive function and learning ability.
