Qualifications/Desired Experience:
Current, active license to practice Physical Therapy in Michigan (or eligibility) Proven clinical experience within a hospital or skilled nursing facility (SNF) setting Strong assessment, treatment planning, and documentation skills Commitment to patient-centered care and team collaboration Excellent communication and organizational abilities
Key Responsibilities:
Conduct thorough physical therapy assessments and craft personalized care plans Deliver evidence-based therapeutic interventions aiming at optimal patient mobility and function Work collaboratively with healthcare team members in a SNF environment Accurately document patient progress and report outcomes Educate patients and caregivers on therapeutic exercises and recovery processes Maintain compliance with facility protocols and state regulations
