JOB OBJECTIVE SUMMARY
The Physical Therapist is a key associate at Hope Network whose primary responsibilities include: Provides physical therapy services as prescribed by consumer's physician for adults and adolescents with head injuries, spinal cordinjuries, chronic pain, general orthopedic rehabilitation needs, and/or neurological behavioral issues. Work with supervision according to Section 333.16109 of the Michigan Public Health Code
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

The employee is required to follow any other instructions and to perform any other duties as assigned by management. Regular and predictable attendance is an essential requirement of this position. Conduct evaluations on consumers as requested by prescribing physicians. Provide physical therapy services which may include: conducting evaluations and assessments, improving strength, balance, endurance, coordination, joint range of motion, muscle flexibility, movement patterns, gait of clients, designing and instructing in execution of, home exercise programs to client and direct care staff, prepare clinical treatment plans and progress notes for clients seen, arranging appropriate external supportive devices and mobility related equipment. Provide regular training's for direct care staff on general consumer transfers and gait exercises. Consult with both direct care staff and professional team members, especially PTA's and Aides, regarding treatment and consumer progress. Conduct trainings, in-services, and participate in team meetings as assigned. Participate in the development and implementation of the program TQO/Quality Improvement plan. Monitor equipment in physical therapy department for needed repair or replacement. Serve as a member of an interdisciplinary team for assigned clients. Participate in client goal development, coordination of services, and discharge planning/family education as needed. Attend and verbally present at team meetings, clinical reviews and in-services. Communicate clearly to team members and interested parties on the Progress, functional status, recommendations and needs of the consumer. Familiarity with all departmental equipment, and ability to implement equipment according to consumer goals/needs. Schedule clients weekly as prescribed and report to supervising therapist any changes in frequency, status and improvements as needed. Participate in the orientation and training of new staff, student interns and volunteers as assigned by the supervisor. Other duties as assigned. Educational /
Talent Requirements:
Bachelors, Masters, or Doctorate degree from an accredited university or institution offering physical therapy curriculum Licensure by examination to practice as a physical therapist Member of the American Physical Therapy Association One-year prior employment experience as a physical therapist Demonstrated ability to work with consumers having special physical, emotional, social, behavioral and vocational needs as a result of neurological disability/TBI Must be willing to work flexible hours on occasion - based on consumer needs Demonstrated ability to design, implement, update and document consumer treatment programs Ability to lift 50 pounds, complete transfers of consumers independently, bend, twist, and kneel to complete job responsibilities Valid driver's license and driving record acceptable according to HNRS policy Ability to articulate and support the mission of HNRS to various audiences
