We're looking for excellent Physical Therapists—clinicians who take pride in their craft, communicate clearly, and show up with purpose. Here, your clinical judgment is respected, your time is protected, and your patients are treated like family. What You'll Do Deliver one-on-one, high-impact physical therapy in the home setting and senior living facility settings (examples: Independent Living, Assisted Living). Practice in a senior-living-centric care model, supporting residents where they live and ensuring care aligns with the community environment and routines. Collaborate with senior living wellness teams and executive teams to drive better health outcomes for residents through shared goals, clear communication, and consistent follow-through. Collaborate with Community Health Navigators to streamline referrals, close loops on care plans, and ensure residents and families have a seamless experience. Partner closely with nurses and therapists through disciplined interdisciplinary collaboration. Drive outcomes with strong education, progression, and safety planning—especially for fall risk, mobility decline, and post-acute transitions. Be a trusted guide for residents and families—providing clarity, confidence, and continuity of care. What We're Looking For We want the best—people who: Take ownership and follow through.
