Home Health Physical Therapist:

Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient's plan of care and goals Observe movements, conduct assessments, document progress and modify care plan as needed Educate patients on their plan of care, established goals, exercises and self-care Coach family on the plan of care, therapy goals and safely assisting with exercises We genuinely care where you are in your career path.
