We are seeking a compassionate and skilled PT to help improve the health and well-being of our clients through tailored therapeutic interventions and support. Responsibilities Conduct comprehensive patient assessments and develop individualized treatment plans. Provide skilled physical therapy services in patients' homes to improve mobility, function, and quality of life. Document patient progress and report on outcomes to healthcare team and insurance providers. Educate patients and family members on exercises, equipment use, and safety precautions. Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care. Requirements Licensed Physical Therapist in the state of Michigan. Proven experience in home health physical therapy preferred. Strong communication and interpersonal skills. Ability to work independently and manage a flexible schedule. Reliable transportation to travel to patient homes in the Southeast MI Region and surrounding areas.
