ProHealth Partners has excellent Full Time opportunities in the Monroe, MI territory. We are in need of a Full time Physical Therapist. • The position rates are based on experience in the home health setting. • Our Care Coordination team provides our therapist with valuable resources such as scheduling and documentation support so that you are able to focus on what matters most, patient care. ProHealth Partners is committed to providing the highest standards of Physical, Occupational and Speech therapy for patients in their homes. With our corporate office located in Perrysburg, Ohio, we currently service Northwest Ohio, the Greater Cleveland, Ohio area, as well as Southeast Michigan. We place a strong value not only on the patients and the agencies we work with but our employees as well.
Job Responsibilities:
Supervision to Asst. therapy staff Communicating daily with office staff to accept new patients, complete paperwork and turn in payroll Keeping open communication with patients and home health agencies to promote a positive and caring experience Facilitating communication with Physical Therapist Assistant to manage patient care Completion of Evaluation, Plan of Care, Reassessment, and Discharge visits and paperwork Attend meetings and case conferencesJob Requirements:
Current Physical Therapist Licensure Bachelor's degree in Physical Therapy Ability to travel to patient's home or facility Strong communication skills Home healthcare experience preferred This position requires daily commute to patients' homes and facilities in a 40-mile radius of Monroe, MI Salary options and full time per visit rates are negotiable #IND123Similar jobs in Monroe, MI
