Position Overview:
We are currently seeking a skilled and compassionate Physical Therapist to join our team of healthcare professionals. The Physical Therapist will play a vital role in helping our clients achieve optimal physical functioning, independence, and overall well-being. This position offers the opportunity to make a meaningful impact on the lives of individuals and families within our community.
Responsibilities:
Conduct comprehensive assessments of patients' physical abilities, functional limitations, and rehabilitation needs. Develop individualized treatment plans and goals in collaboration with patients, caregivers, and other members of the interdisciplinary team. Implement evidence-based physical therapy interventions and techniques to improve mobility, strength, balance, and coordination. Provide skilled therapeutic exercises, manual therapy, gait training, and other modalities to optimize functional outcomes. Educate patients and caregivers on safe and effective techniques for maximizing mobility, preventing injury, and promoting overall wellness. Monitor and evaluate patients' progress, adjusting treatment plans as needed to achieve desired goals and outcomes. Document all patient care activities, assessments, interventions, and outcomes accurately and timely in accordance with regulatory standards and agency policies. Communicate effectively with physicians, nurses, and other healthcare professionals to coordinate care, share information, and optimize treatment outcomes. Advocate for patients' rights and preferences, promoting a culture of respect, dignity, and autonomy. Participate in interdisciplinary team meetings, case conferences, and continuing education opportunities to enhance clinical skills and stay abreast of advances in physical therapy practice.
Qualifications:
Valid and current licensure as a Physical Therapist (PT) in the state of Michigan. 1-2 years of experience as a Physical Therapist, with previous experience in home health care preferred. Demonstrated proficiency in clinical skills, including assessment, treatment planning, and implementation of physical therapy interventions. Strong interpersonal and communication skills, with the ability to establish rapport with patients, families, and caregivers. Compassionate and empathetic attitude towards clients and their families, with a commitment to delivering person-centered care. Ability to work independently and collaboratively as part of a multidisciplinary team, prioritizing tasks and managing time effectively. Reliable transportation and willingness to travel to patients' homes within the designated service area. CPR certification and other relevant certifications may be required.
Pay:
$70.00 - $125.00 per hour
