A Home Health Physical Therapist delivers skilled rehabilitation services in patients' homes, focusing on improving mobility, restoring function, reducing pain, and increasing independence. Care is tailored to each patient's condition and living environment, with an emphasis on practical, real-world recovery.
Core Responsibilities:
Perform in-home evaluations to assess physical function, mobility, and safety needs Develop and implement individualized treatment plans based on clinical findings and patient goals Provide therapeutic interventions such as exercise programs, gait training, balance training, and functional mobility work Evaluate home environments for safety risks and recommend adaptive equipment or modifications Educate patients and caregivers on safe movement techniques and home exercise programs Track patient progress and modify treatment plans as needed Document all care accurately and in compliance with regulatory and agency standards Collaborate with physicians, nurses, and other members of the healthcare team Travel between patient homes within assigned service areas
Minimum Qualifications:
Doctor of Physical Therapy (DPT) or equivalent degree Active Physical Therapist license in the practicing state Valid driver's license and dependable transportation CPR certification (or willingness to obtain)
Preferred Experience:
Prior experience in home health, outpatient, or geriatric rehabilitation
Key Skills:
Strong clinical judgment and ability to work independently Excellent communication and patient education skills Ability to adapt treatment plans to varied home environments Effective time management and organizational skills Compassionate, patient-centered approach to care
Work Conditions:
Daily travel to patient residences Variable home environments and patient needs Flexible schedule based on caseload
