PRN Home Health Physical Therapist New Dimensions Home Health Care - Fergus Falls Area Flexible PRN Opportunity New Dimensions Home Health Care is seeking a compassionate and motivated PRN Home Health Physical Therapist to join our growing team in the Fergus Falls area. This is an excellent opportunity for a licensed Physical Therapist who enjoys providing personalized, one-on-one care while helping clients improve mobility, independence, and overall quality of life in the comfort of their own homes. In this role, you will use your clinical expertise to evaluate, develop, and implement individualized treatment plans designed to restore function, reduce pain, and prevent disability while supporting clients in achieving their highest level of independence. What You'll Do Evaluate clients and assess physical therapy needs Develop and implement individualized treatment plans Help clients restore function, mobility, strength, and independence Provide therapeutic exercises and rehabilitation services Educate clients and caregivers on treatment plans and home exercises Monitor progress and adjust treatment plans as needed Maintain accurate and timely documentation Collaborate with healthcare providers and care teams to support optimal client outcomes What We're Looking For Current Physical Therapist license required Strong clinical assessment and treatment planning skills Excellent communication and interpersonal abilities Compassionate, patient-focused approach to care Ability to work independently and manage time effectively Home health experience preferred, but not required Why Join New Dimensions Home Health Care?
