Deliver therapeutic interventions to improve strength, balance, and mobility. Supervise and collaborate with Physical Therapist Assistants. Coordinate care with nurses, therapists, and physicians. Monitor patient progress and update plans as needed. Complete documentation including OASIS when required. Physical Therapist |

Qualifications Licensed Physical Therapist in the State of Minnesota. Home care experience preferred. OASIS experience preferred. Ability to manage schedule and caseload independently. Valid driver's license and reliable vehicle. Strong clinical judgment and patient communication skills.
