What You'll Be Doing Perform physical therapy evaluations and treatment visits in patient homes. Develop treatment plans focused on strength, mobility, balance, endurance, and functional recovery. Help patients safely transition following hospitalization, surgery, illness, or injury. Educate patients and caregivers on exercises, safety strategies, and long-term success. Coordinate care with nurses, occupational therapists, speech therapists, physicians, and case managers. Complete documentation supporting patient outcomes and regulatory requirements. What We're Looking For Current Minnesota Physical Therapist (PT) license. Strong clinical assessment and patient education skills. Home health experience preferred but not required. Ability to work independently and manage a visit-based schedule. Reliable transportation, valid driver's license, and current auto insurance.
