Job Description
Job Summary We are seeking a dedicated and energetic Physical Therapist to join our dynamic healthcare team. In this vital role, you will assess, develop, and implement personalized care plans to improve patients' mobility, strength, and overall well-being. Your expertise will support patients across various settings—including inpatient hospital units, outpatient clinics, home health environments, and community care programs—empowering them to achieve their rehabilitation goals. This position offers an exciting opportunity to make a meaningful impact on patients' lives through compassionate care and innovative physiotherapy techniques. Responsibilities Conduct comprehensive patient assessments to evaluate physical function, mobility, and pain levels using detailed physiology and anatomy knowledge. Develop and implement tailored care plans that incorporate evidence-based physiotherapy practices such as trigger point therapy and orthopedics. Deliver hands-on therapy sessions focused on improving strength, flexibility, and functional independence for diverse patient populations including pediatrics, geriatrics, and those recovering from injury or surgery. Coordinate discharge planning by collaborating with multidisciplinary teams to ensure seamless transition from hospital to home or community care settings. Utilize electronic medical records (EMR) systems efficiently to document patient progress, treatment plans, and clinical notes accurately. Educate patients and their families on injury prevention, home exercise programs, and self-care strategies to promote ongoing recovery outside clinical environments. Stay current with advancements in physiotherapy techniques and maintain compliance with healthcare standards through continuous professional development. Requirements Valid licensure as a licensed Physical Therapist in the relevant jurisdiction with current registration. Proven experience in acute care hospital settings, outpatient clinics, or home health environments demonstrating strong patient care skills. In-depth understanding of medical terminology, physiology, and human anatomy essential for accurate assessment and treatment planning. Experience working with diverse populations including pediatrics, geriatrics, orthopedics, and patients requiring inpatient or outpatient rehabilitation services. Familiarity with EMR systems for efficient documentation and patient record management. Knowledge of discharge planning processes and ability to coordinate care across inpatient, outpatient, home & community care settings. Pay:
