Not required Role Overview This role focuses on delivering high-quality in-home physical therapy services including evaluations, reassessments, and start-of-care (SOC) visits. The clinician may also support treatment visits as needed, working within a home health model focused on patient recovery, mobility improvement, and functional independence. Key Responsibilities Perform physical therapy evaluations, reassessments, and SOC visits in home health settings Deliver treatment visits based on patient care plans Provide patient-centered rehabilitation focused on mobility, strength, and functional oues Maintain timely and accurate clinical documentation (24-hour sync-back requirement) Ensure high-quality,pliant, and thorough visit documentation Collaborate with interdisciplinary care teams to support continuity of care Travel within assigned 50-mile service radius to patient homes Ad to home health protocols, safety standards, and regulatory requirements Must-Have Qualifications Active Physical Therapy license in the state of Arkansas Current CPR certification Prior home health experience required Experience using HCHB (Homecare Homebase) system Preferred Qualifications Strong efficiency in clinical documentation and workflow management Experience with SOC visits, evaluations, and reassessments Ability to manage a full home health caseload independently Strongmunication and patient education skills Ability to deliver high-quality care under productivity expectations Additional Notes Blue scrubs preferred Strong emphasis on documentation turnaround (24-hour requirement) Ideal for PTsfortable with independent, field-based work
Pay Details:
