On-Call/Weekend Requirement:

One weekend per month (if possible)

Mileage:

Reimbursed between patient visits Responsibilities include but are not limited to:

• Evaluate and treat patients in a homecare setting
• Develop and implement individualized treatment plans
• Provide skilled physical therapy services across assigned coverage areas
• Document patient care accurately and in a timely manner
• Travel between patient homes throughout Morris and Essex counties
• Maintain productivity expectations (30 points per week)
• Collaborate with care teams to ensure continuity of care

Qualifications:

• Active New Jersey Physical Therapist (PT) license required
• Minimum 2-3 years of homecare, acute, or subacute experience required
• Strong clinical assessment and treatment skills
• Excellentmunication, accountability, and patient care skills
• Ability to manage independent travel-based caseload

Pay Details:
