What You'll Do Complete physical therapy evaluations and treatment visits in patient homes. Develop individualized treatment plans focused on mobility, strength, balance, endurance, and fall prevention. Help patients recover from hospitalization, surgery, injury, and chronic conditions. Recommend assistive devices, home modifications, and safety strategies. Educate patients and caregivers to maximize long-term success. Collaborate closely with nurses, occupational therapists, speech therapists, physicians, and case managers. Complete documentation that supports quality outcomes and continuity of care. What We're Looking For Current Minnesota Physical Therapist (PT) license. Strong clinical assessment and treatment planning skills. Home health experience preferred but not required. Ability to work independently and manage a visit-based schedule. Reliable transportation, valid driver's license, and current auto insurance.
