Responsibilities Provide in-home physical therapy evaluations and treatment visits. Complete admissions and start of care assessments. Develop individualized treatment plans based on patient goals. Deliver therapeutic interventions to improve strength, balance, and mobility. Supervise and collaborate with Physical Therapist Assistants. Coordinate care with nurses, therapists, and physicians. PT / DPT

Qualifications Licensed Physical Therapist in the State of Minnesota. Home care experience preferred. OASIS experience preferred. Ability to manage schedule and caseload independently. Valid driver's license and reliable vehicle. Strong clinical judgment and patient communication skills.
