Full Time Home Health Physical Therapist

Deliver patient-centered care in home settings with a Monday‑Friday schedule and no weekend requirement. Opportunity for growth in a supportive team environment.
Key Responsibilities:
Perform comprehensive patient evaluations and develop individualized treatment plans. Conduct start‑of‑care assessments and complete OASIS documentation accurately. Provide routine home‑health therapy visits focused on strength, balance, mobility, and independence. Complete patient recertifications and reassessments as required. Educate patients and caregivers on therapeutic exercises, mobility strategies, and home safety. Collaborate with physicians, nurses, case managers, and interdisciplinary teams. Monitor patient progress and modify treatment plans to maximize outcomes. Maintain timely and accurate clinical documentation using Matrix EMR. Deliver compassionate, patient‑centered care in a home‑based setting.
Qualifications:
Doctorate or Master's degree in Physical Therapy from an accredited program. Active Missouri Physical Therapist license required. Strong communication, organization, and time‑management skills. Ability to work independently and manage a daily patient schedule.
Preferred Skills:
CPR/BLS certification. Experience with OASIS documentation and Matrix EMR.
