Schedule:
24 hrs./week | 8:00 AM and 4:30 PM Emergency services are not required. Job Overview We are seeking a dedicated and compassionate Part-time Physical Therapist to join our healthcare team. The Physical Therapist will provide professional physical therapy services to consumers as directed by the state agency. Services will include evaluation, treatment planning, direct and indirect therapy services, documentation, family education, and staff training. The Physical Therapist will support consumers in improving mobility, strength, endurance, balance, coordination, cardiopulmonary function, joint mobility, flexibility, and functional abilities needed for daily living. Duties The Physical Therapist shall be responsible for providing physical therapy services through various modalities, including but not limited to heat, cold, light, air, water, sound, electricity, massage, mobilization, bronchopulmonary hygiene, exercise, and gait training with or without assistive devices.
Responsibilities include:
Performing initial physical therapy evaluations and assessments for each consumer requiring physical therapy services. Formulating individualized treatment plans based on the initial assessment, including goals and objectives aimed at correcting or reducing disability or pain and restoring lost function. Providing direct physical therapy services as directed by the state agency, focusing on posture, locomotion, strength, endurance, cardiopulmonary function, balance, coordination, joint mobility, flexibility, and functional abilities in daily living skills. Providing indirect physical therapy services, including routine monitoring and documentation of consumer progress following each assessment and therapy session. Preparing recommendations and documenting progress at each physical therapy session. Providing in-service training to state agency personnel, as requested, in areas including assessment and training in locomotion and the use of orthotic, prosthetic, and/or assistive devices. Providing physical therapy education and instruction to consumers and their families, as requested by the state agency. Coordinating services in accordance with state agency direction, professional standards, and applicable documentation requirements.

The candidate must be a qualified Physical Therapist with appropriate licensure, credentials, and professional experience to perform physical therapy services in accordance with applicable state requirements and agency standards.
