Job Description

Degree in Physical Therapy, active Physical Therapist license and 1+ year of Physical Therapy experience required. Applicants who do not meet these qualifications will not be considered. Provide specialized physical therapy services to patients recovering from physical functioning disorders and wound care at a healthcare facility in Oregon, MO. This contract position involves comprehensive patient assessment, development and implementation of individualized rehabilitation plans, and collaboration with multidisciplinary teams to optimize patient outcomes.
Key Responsibilities:
Conduct thorough patient screenings and evaluations using standardized assessments and evidence-based practices. Develop, implement, and adjust treatment plans including manual techniques, ambulation, therapeutic exercises, modalities, assistive devices, and wound care. Monitor patient progress, evaluate treatment effectiveness, and modify services accordingly. Document all treatments, patient progress, and outcomes accurately in the electronic health record system in compliance with regulatory standards. Provide consultation and counseling to patients, families, caregivers, and other healthcare providers. Collaborate with healthcare team members through participation in patient care conferences, utilization review meetings, and rehabilitation meetings. Prepare comprehensive discharge summaries following regulatory and clinical guidelines. Obtain necessary durable medical equipment and supplies as required. Supervise and support physical therapist assistants, rehabilitation aides, and students. Adhere to APTA standards and State practice acts, maintaining professional ethics and conduct throughout.
