8:00 AM-5:00 PM, Monday-Friday Start date: 06/22/2026 Work setting: hospital outpatient department (95% outpatient, ~5% LTC/Skilled Swing) Extension opportunities: strong possibility for longer assignment Responsibilities Perform initial evaluations and develop individualized physical therapy treatment plans Implement interventions including therapeutic exercise, gait and balance training, manual therapy, and functional mobility activities Reassess patient progress and adjust plans to optimize functional goals Educate patients and families on home exercise programs, safety, and self-management strategies Document assessments, plans of care, progress notes, and communicate with the interdisciplinary care team Collaborate with other health professionals and ancillary staff to coordinate patient care and discharge planning Maintain professional standards and ensure patient safety and comfort throughout treatment Requirements Ability to work in an outpatient/hospital-based setting with a broad patient mix Strong communication and teamwork skills Willingness to adapt to a community-focused, collaborative approach Commitment to a 13-week travel assignment with potential for extension Capacity to treat a wide range of patients and adapt treatment plans accordingly Benefits and Support 13-week assignment with potential extension 40 guaranteed hours per week Start date: 06/22/2026
