Implement evidence-based interventions addressing mobility, gait, strength, balance, pain management, and activities of daily living. Monitor patient progress, adjust goals, and document outcomes in collaboration with the interdisciplinary team. Administer appropriate therapeutic modalities and ensure safety, alignment with geriatric care protocols, and infection control standards. Educate patients and families on home exercise programs, fall prevention, and discharge planning. Maintain accurate, timely documentation and participate in care planning and transitions.
