Proudly supported by Marquis Health Consulting Services Per diem opportunity available•1 day a week! Same Day Pay! Flexible scheduling, no productivity mandates, top pay rates and weekend rates! Responsibilities as Physical Therapist (PT): Assists the physician in evaluating the patient's level of function, as well as assess patient's home environment for safety hazards and architectural barriers and identifies the patient's durable medical equipment needs. Identifies functional problems, establishes goals and a plan of care designed to improve the patient's functional status (revising as necessary). Performs patient tests, measurements, and evaluations, such as range of motion and manual muscle tests, gait and functional analysis, and body parts measurement, and records and evaluates findings to aid in establishing or revising specifics of treatment programs. Performs and completes start-of-care, recertifications, resumption-of-care, and discharge Comprehensive OASIS assessments and associated paperwork in accordance with agency policy. Instructs patient and family members, as well as other health team members, during certain phases of physical therapy in which they may work with the patient. Periodically re-evaluates and upgrades the patient's program to maximize functional improvement. Administers the exercise program utilizing specialized therapeutic exercise techniques and equipment. Plans and administer prescribed physical therapy treatment programs for patients to restore function, relieve pain and prevent disability following disease, injury, or loss of body part. Confers with the family, referring physician, and other agency professional staff to obtain information to promote coordinated, efficient, and quality patient care. Maintains necessary statistical and billing data. Performs physical therapy assistant on-site supervision as required by law. Qualifications for Physical Therapist (PT): Must be a graduate from an accredited Physical Therapy program and licensed by the state of practice who meets the qualifications under the Federal conditions of participation for Home Health Agencies established by CMS.
