Job Description

Overview Join our dynamic healthcare team as a dedicated Physical Therapist (PT), where your expertise will transform patient recovery journeys across diverse settings. In this vital role, you will assess, develop, and implement personalized care plans that promote mobility, reduce pain, and enhance overall well-being. Your passion for physiotherapy and commitment to patient-centered care will drive positive outcomes in our outpatient clinic. Responsibilities Conduct comprehensive assessments of patients' physical conditions, including physiology and anatomy knowledge, to determine appropriate treatment strategies. Develop and implement tailored care plans that incorporate physiotherapy techniques such as trigger point therapy and orthopedics management. Provide hands-on therapy sessions focusing on improving mobility, strength, and function for geriatric population. Coordinate discharge planning to ensure seamless transition to HEP. Utilize electronic medical records (EMR) systems efficiently to document progress notes, treatment plans, and patient outcomes accurately. Educate patients and their families on injury prevention, rehabilitation exercises, and self-care strategies to promote long-term health benefits. Requirements Valid licensure as a Physical Therapist (PT) in the relevant jurisdiction with current registration. Proven experience in outpatient setting and desire to treat people, not parts. Strong knowledge of physiology, anatomy, and medical terminology essential for accurate assessment and treatment planning. Demonstrated ability to develop effective care plans tailored to individual patient needs in geriatric population. Experience with EMR systems for documentation and patient management; familiarity with home & community care is a plus. Skilled in various physiotherapy techniques including trigger point therapy, orthopedics rehabilitation, and geriatrics-specific interventions. Excellent communication skills to educate patients effectively while working collaboratively within multidisciplinary teams. Pay:
$42.03 - $50.62 per hour
