Job Description

Job Title:
Physical Therapist (PT) Position Summary The Physical Therapist (PT) is responsible for evaluating, planning, and implementing individualized treatment programs for residents in a skilled nursing facility. The PT focuses on restoring function, improving mobility, relieving pain, and preventing or limiting permanent physical disabilities. This role ensures high-quality care delivery while maintaining compliance with regulatory and reimbursement guidelines. Key Responsibilities Perform comprehensive evaluations of residents' physical conditions, functional limitations, and rehabilitation needs Develop and implement individualized treatment plans based on clinical findings and physician orders Provide therapeutic interventions including exercise, manual therapy, gait training, balance training, and modalities Supervise and direct Physical Therapist Assistants (PTAs) and rehabilitation aides Monitor resident progress and modify treatment plans as needed to achieve optimal outcomes Complete accurate and timely documentation, including evaluations, progress notes, recertifications, and discharge summaries Ensure compliance with Medicare (Part A & B), PDPM guidelines, and state/federal regulations Participate in interdisciplinary team meetings, care planning, and discharge planning Educate residents, families, and caregivers on treatment plans, safety, and home exercise programs Maintain productivity standards while delivering quality patient care Ensure a safe and effective therapy environment Qualifications Doctor of Physical Therapy (DPT) or Bachelor's/Master's degree in Physical Therapy from an accredited program Current Physical Therapist license in the state of practice Previous experience in a skilled nursing facility or geriatric setting preferred Strong knowledge of rehabilitation techniques, equipment, and safety protocols Familiarity with Medicare regulations, PDPM, and documentation requirements Excellent communication, clinical reasoning, and interpersonal skills Ability to supervise and mentor support staff Work Environment Skilled nursing / long-term care setting Frequent standing, walking, bending, and assisting residents with mobility May require lifting or supporting residents using proper body mechanics and equipment Fast-paced environment with productivity expectations Key Competencies Clinical expertise and decision-making Patient-centered care Leadership and supervision Time management and organization Collaboration and teamwork Supervision Reports to: Director of Rehabilitation /
