Job Description

Physical Therapist (PT) - Hospice & Home Healthcare of Saunders County About Us We are a compassionate, patient-centered hospice and home health organization dedicated to providing high-quality care to individuals and families during some of life's most challenging moments. Our interdisciplinary team works collaboratively to support patients' physical, emotional, and psychosocial needs in the comfort of their homes. As a small, locally owned hospice and home health agency, we pride ourselves on caring for our patients the same way we would care for our own family. Position Summary We are seeking a dedicated and compassionate Physical Therapist (PT) to join our hospice and home health team. In this role, you will evaluate patients, develop individualized treatment plans, and provide skilled physical therapy services designed to improve mobility, reduce pain, restore function, and promote independence in the comfort of patients' homes. You will play a vital role in enhancing patients' quality of life through compassionate, patient-centered care and interdisciplinary collaboration. Key Responsibilities Evaluate patients and develop individualized physical therapy treatment plans Provide skilled physical therapy interventions to improve mobility, strength, balance, endurance, and functional independence Assess patient progress and modify treatment plans as needed Educate patients and caregivers on therapeutic exercises, fall prevention, safety techniques, mobility training, and use of assistive devices Collaborate with physicians, nurses, social workers, aides, and other interdisciplinary team members to coordinate patient care Maintain accurate, timely, and compliant clinical documentation Participate in care planning, case conferences, and discharge planning as appropriate Observe infection control, patient safety, and confidentiality protocols at all times Provide compassionate support and encouragement to patients and families throughout the plan of care Supervise Physical Therapist Assistants (PTAs) when applicable and in accordance with state regulations Qualifications Graduate of an accredited Physical Therapy program Current state Physical Therapist license in good standing CPR certification Minimum of 1-2 years of experience in home health, hospice, rehabilitation, or healthcare preferred Strong clinical assessment and communication skills Ability to work independently and collaboratively within an interdisciplinary team Compassionate, patient-focused approach to care Valid driver's license and reliable transportation What We Offer Competitive compensation Flexible scheduling options Supportive team environment PTO and sick time beginning immediately Retirement match after one (1) year of employment How to Apply Interested candidates are encouraged to submit their resume and a brief cover letter outlining their experience and passion for hospice and home health care.
