Job Summary:
Seeking a compassionate and skilled Physical Therapist (PT) to provide high-quality therapy services to patients across all age groups. The ideal candidate will develop and implement individualized treatment plans, promote functional recovery, and collaborate with interdisciplinary healthcare teams to achieve optimal patient outcomes.
Roles and Responsibilities:
Evaluate patients and develop individualized, age-appropriate treatment plans based on clinical findings. Provide skilled physical therapy services to patients across all age groups. Plan, implement, and manage physical therapy care plans to improve mobility, function, and quality of life. Reassess patient progress and modify treatment plans as needed. Apply evidence-based physical therapy practices to support patient recovery and rehabilitation. Educate patients and caregivers regarding treatment plans, exercises, and wellness strategies. Communicate effectively with physicians, healthcare teams, patients, and families. Participate in quality improvement initiatives and problem-solving activities to enhance patient care. Supervise, orient, and mentor Physical Therapist Assistants (PTAs) in accordance with state regulations and organizational policies. Maintain accurate and timely clinical documentation.

Active New Hampshire Physical Therapist (PT) License required. Bachelor's Degree from an American Physical Therapy Association (APTA) accredited Physical Therapy program required. Minimum one (1) year of healthcare experience performing similar duties required. Strong knowledge of current physical therapy practices and patient care standards. Ability to work effectively in a multidisciplinary healthcare environment. Excellent organizational, communication, and problem-solving skills. AL01
