Responsibilities Develops interventions to support goals of therapy and restore maximum function and strength promoting optimal health and minimal pain. Evaluates results of physical therapy by evaluating by observing, noting, and evaluating patient's progress; recommends and implements adjustments as needed. Completes patient admission to agency's services after thorough review of medical records. Assures continuation of therapeutic plan following discharge designing and instructing patients, families, and caregivers in home exercise programs. Qualifications Bachelor's degree in Physical Therapy. A minimum of two years therapy experience, either hospital or clinic experience. Preferred Qualifications Master's or Doctorate degree in Physical Therapy. Homecare Experience. Required Registered as Physical Therapist with New Hampshire's Board of Nursing. CPR Certification.
