Compensation:
$110,000•$125,000 annually + performance-based bonus

Position Summary We are currently seeking a Full-Time Physical Therapist (PT/RPT) to join our growing home care team. This role provides consistent full-time hours (40 hours per week), competitive compensation, comprehensive benefits, performance-based bonuses, and the opportunity to make a meaningful impact by delivering one-on-one care in patients' homes. The Physical Therapist will support patients across Lowell, Chelmsford, Framingham, Billerica, and surrounding communities along the I-495 belt, working closely with our skilled nursing and interdisciplinary care teams. Key Responsibilities Provide comprehensive and compassionate physical therapy services in patients' homes in accordance with physician orders Perform initial evaluations and ongoing assessments of patients' functional abilities Develop and implement individualized treatment plans to improve mobility, strength, balance, and quality of life Deliver skilled therapeutic interventions, exercises, and patient/caregiver education Document patient progress accurately and in a timely manner per company and regulatory standards Collaborate with nurses, caregivers, physicians, and interdisciplinary teams to ensure coordinated care Maintain a safe, supportive, and professional therapy environment at all times Qualifications Graduate of an accredited Physical Therapy program Current Physical Therapist (PT/RPT) license in Massachusetts Prior experience in home care of 2 years, skilled nursing, or sub-acute rehab preferred but not required Strong communication, interpersonal, and clinical documentation skills Ability to work independently and manage a full-time caseload Reliable transportation and ability to travel within assigned service areas What We Offer Full-time schedule (40 hours/week) Competitive salary: $110,000•$125,000 annually Performance-based bonus opportunities Health insurance benefits Paid time off and holidays Flexible scheduling and work-life balance Supportive leadership and clinical team environment Company perks and growth opportunities
