  • Per diem Edgewood Center for Rehab Edgewood Center for Rehab is seeking a motivated and compassionate Per Diem Physical Therapist (PT) to join our rehab team.
Our beautiful 5-star skilled nursing facility in historic Portsmouth provides exceptional short-term rehabilitation and long-term care services in a supportive, team-focused environment. Qualifications Graduate of an accredited Physical Therapy program Current NH Physical Therapist license in good standing Long-term care and/or geriatric experience preferred Position Summary The Physical Therapist evaluates residents, develops individualized treatment plans, and provides skilled therapy services focused on restoring function, improving mobility, reducing pain, and maximizing independence. Responsibilities Evaluate residents and establish individualized treatment plans Provide skilled physical therapy interventions and therapeutic exercises Complete accurate and timely documentation Collaborate with interdisciplinary team members to support resident outcomes Monitor resident progress and modify treatment plans as appropriate Educate residents, families, and staff on mobility, safety, and exercise programs Assist with discharge planning and equipment recommendations Maintain a safe, clean, and organized therapy environment Support a positive and collaborative rehab culture Why Join Us?
