Physical Therapist- Full time

We are seeking a compassionate and motivated Full-Time Physical Therapist to join our skilled nursing team. The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide therapy services to improve mobility, strength, balance, and overall quality of life. Responsibilities Evaluate residents' physical abilities and therapy needs Develop and implement individualized treatment plans Provide therapy focused on mobility, strength, balance, pain management, and functional independence Collaborate with nursing staff, physicians, and interdisciplinary teams Educate residents and families on exercises and safety techniques Complete accurate EMR documentation and progress notes Assist with discharge planning and care transitions as needed Qualifications Licensed Physical Therapist in the state of employment Skilled nursing, rehab, or long-term care experience preferred Strong communication and patient care skills Experience with EMR documentation systems preferred Ability to work collaboratively in a fast-paced healthcare environment
