COPY LINK link Adecco Healthcare & Life Science is working with our client to hire Travel Physical Therapists (PT) in Nutley, NJ area.. This is an in-person/on-site role providing care in patients' homes within the surrounding service area.

Type:

Six-Month Contract- possibility of extension

Schedule:

Monday-Friday, 8:00 AM - 4:30 PM (No Call)

Pay Rate:

$55 to $59/hour, based on experience

Coverage Areas:

Newark, Nutley, Bellvile and Bloomfield

Primary Responsibilities Include:

• Perform Start of Care (SOC) visits to assess patient needs, develop individualized care plans, and establish treatment goals.
• Conductprehensive physical therapy evaluations to determine functional limitations, mobility status, and rehabilitation potential.
• Provide ongoing therapy visits (revisits) focused on improving strength, balance, mobility, and overall functional independence.
• Complete reassessments and re-evaluations to track patient progress, update care plans, and adjust treatment goals as needed.
• Manage patient discharges, including final evaluations, patient/caregiver education, and transition planning.

Required Qualifications:

• Bachelor's degree in Physical Therapy (or higher) from an accredited program.
• Active Physical Therapist license in the state of practice.
• Prior experience with Home Health Care Home Base (HCHB) and/or PointCare documentation systems.
• Strong clinical assessment, documentation, and patient care skills.

Pay Details:
