Join our Oceanport, New Jersey team! We are seeking a dedicated Physical Therapist to provide high-quality, patient-centered care for older adults in a collaborative PACE setting. This full-time opportunity offers a competitive salary of $100,000 - $115,000 and the chance to make a meaningful impact in helping participants maintain independence and quality of life within their communities. Requirements to Apply Advanced degree in Physical Therapy from an accredited program Current Physical Therapist license in the state of New Jersey Valid driver's license and proof of auto insurance Experience working with interdisciplinary teams and/or elderly or frail populations preferred Job Overview Our client is seeking a compassionate and motivated Physical Therapist to join an interdisciplinary PACE team focused on helping older adults remain safe, independent, and active in their communities. In this role, you will perform evaluations, develop individualized treatment plans, provide hands-on therapy services, assess mobility and home safety needs, and educate participants and caregivers on rehabilitation and wellness strategies. You will collaborate closely with physicians, nurses, social workers, and other care team members to coordinate comprehensive care plans and improve participant outcomes. The ideal candidate is patient-focused, team-oriented, organized, and passionate about serving the elderly population through a holistic, community-based care model.
