No Weekends Position Overview We are seeking a compassionate and skilled Physical Therapist (PT) to join a Home Health team on a 13-week contract assignment supporting patients throughout the Morris County region. The Physical Therapist will be responsible for evaluating, developing, and implementing individualized treatment plans to help patients improve mobility, restore function, manage pain, and achieve optimal independence within their home environment. Responsibilities Conduct comprehensive physical therapy evaluations and assessments in patients' homes Develop individualized treatment plans based on patient needs, physician orders, and clinical findings Implement therapeutic interventions designed to improve strength, balance, mobility, endurance, and functional independence Monitor patient progress and modify treatment plans as appropriate Educate patients, families, and caregivers on exercises, safety precautions, mobility techniques, and home exercise programs Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams to coordinate patient care Complete accurate and timely clinical documentation in accordance with regulatory and organizational standards Participate in care planning and discharge planning activities Ensure compliance with home health regulations, professional standards, and agency policies Promote patient safety and quality outcomes throughout the course of treatment Requirements Graduate of an Accredited Physical Therapy Program Doctor of Physical Therapy (DPT) or Equivalent Physical Therapy Degree Current and Unrestricted Physical Therapist (PT) License Strong clinical assessment and treatment planning skills Knowledge of rehabilitation techniques, mobility training, and patient education strategies Excellent communication and interpersonal skills Strong organizational and time-management abilities Ability to work independently while managing a regional caseload Commitment to patient-centered care and quality outcomes Preferred Home Health Experience Experience providing rehabilitation services in community or patient home settings Experience with Electronic Medical Records (EMR) Epic EMR Experience Knowledge of Medicare and Home Health Regulations For more details contact at or Call / Text at 516-862-1169.
