1 Description American Traveler is seeking a licensed Physical Therapist for a 13-week home health assignment in the Albuquerque, NM area, with a Monday-Friday schedule and no prior home health experience required. Details Home health setting serving patients within a 45-mile radius of Albuquerque, including surrounding communities such as Tijeras, Rio Rancho, Bernalillo, Bosque Farms, Los Lunas, and Belen Patient caseload includes initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications per Medicare and state standards Services include skilled interventions, maintenance therapy programs, and OASIS assessments in accordance with state-defined scope of practice Schedule is Monday through Friday with no weekend shifts indicated 13-week contract Requirements Graduate of an accredited physical therapist program approved by CAPTE, APTA, or CAHEA Current, active PT license in the state of practice required; license required if applicable Current CPR certification required Two years of clinical PT experience preferred; home health experience is a plus but not required Basic computer proficiency and willingness to learn EMR systems and OASIS documentation Strong clinical judgment and ability to work independently with minimal oversight Reliable transportation, valid driver's license, and personal auto insurance required for home visits DOB and SSN may be required for consideration Additional Information Responsibilities include developing and updating individualized plans of care, delivering skilled interventions, supervising PTAs and Home Health Aides, and coordinating with the interdisciplinary care team OASIS assessments, recertifications, transfers, resumptions of care, and discharges must be submitted within agency-defined timeframes Accurate ICD-10 coding and timely clinical documentation are expected Candidates will periodically present in-service programs and contribute to staff education initiatives Active participation in Quality Assessment and Performance Improvement (QAPI) activities is expected Position requires adherence to HIPAA and all agency privacy and confidentiality standards Pay rate is all-inclusive and does not include additional mileage reimbursement for travel to patient homes
