Salary Pay Range:
$64,480•72,800 Physical Therapist is responsible for evaluating, developing, and implementing physical therapy services. Documents treatment sessions in accordance with policies and procedures of the department and regulatory bodies. Supervises Physical Therapist Assistants in accordance with NYS Department of Education and other regulatory bodies.
ESSENTIAL FUNCTIONS/RESPONSIBILITIES
Completes an initial evaluation for all newly referred patients. Develops treatment plan based on evaluation and assessment. The treatment plan is submitted to the medical director and/or referring provider for approval or certification. Modifies treatment plan as deemed necessary. Provides direct physical therapy services and documents treatments as required. Provides clinical supervision to Physical Therapy Assistants in accordance with NYS Practice Guidelines and Insurance Guidelines. As a member of the Interdisciplinary treatment Team, participates in professional meetings as assigned, integrating services with other providers. Makes recommendations for the purchase of specialized equipment, materials, and wheelchair modifications. Lends counsel to patients and their family members/care givers (if applicable) regarding the patient's Physical Therapy Services including staff/family education upon discharge. Provides an orientation to physical therapy services for new staff and participates in in-service training. Responsible for ensuring that confidentiality is strictly maintained concerning all agency-related information. Maintains productivity expectations set by the rehab leadership team. Functions as a clinical instructor for PT/PTA students (after a minimum of 1 year's work experience). Maintains clinical competency by attending and completing at least one continuing education class/course each year as well as maintaining CEU requirements for state licensure. Maintains 90% compliance on Chart Audits. Follows required documentation guidelines and expectations set forth by the rehab department and regulatory bodies (Medicare, Medicaid, NYS Ed, DOH, Commercial Insurances, etc.). Refers patients to other disciplines if necessary. Maintains SCIP-R Core Certification and CPR/AED Certification as well as all required certifications set in place by rehab leadership team Covers physical therapy services at 800 Hertel and/or Day Programs as assigned Complies with all agency policies and procedures. Other duties as assigned.

Adapts approach as situations change and accepts changes in the organization and job description requirements. Supports the clinic and agency by accepting additional tasks outside of job description such as special projects, and public education. Plans time off in advance and manages PTO according to department guidelines.
MINIMUM QUALIFICATIONS
Doctorate degree from an accredited Physical Therapy Program or Licensed and currently registered by New York State Department of Education to practice physical therapy
