Your responsibilities include evaluating and treating geriatric patients in their homes, developing individualized care plans, and documenting all care using Homecare Homebase EMR. You will educate patients and families on mobility and safety, collaborate with interdisciplinary teams, and ensure patient progress toward therapy goals. No mileage reimbursement is provided.
