Purpose:
Following a medically prescribed physical therapy program, evaluates, treats and educates pediatric, adolescent, adult or geriatric patients who have disabilities, traumatic injuries, pain, disease or any dysfunction, and helps them reach their maximum performance and assume a place in their community while learning to live within the limits of his/her capabilities. Demonstrates knowledge and skills necessary to provideappropriate careto the age of the patient served in the inpatient and/or outpatient setting. - Demonstrates regular,reliableand predictable attendance.
- Performs other duties asrequired.
- Provides direct patient care services to ensure the effectiveness of physical therapy programs.
- Appropriate evaluates patients andestablishesan effective treatment plan to achieve set goals.
- Handlesconsistentlyan assigned patient caseload and provides treatment to all patients within the assigned caseload.
- Independently prioritizes caseload and available treatment times toprovide formaximum efficiency.
- Utilizes correct modalities of treatment and suggests variations.
- Recognizes contraindications to continuing with scheduled treatment and consuls with responsible Physician.
- Involves family or caregivers in treatment plan and discharge planning.
- Respects patient privacy, confidentiality of patient information andexhibitsbehavior consistent with that of a professional health care provider.
- Interprets theappropriate informationneeded toidentifyeach patient's requirementsrelativeto his or her age-specific needs.
- Provides the care needed as described in the Department's policies and procedures.
- Demonstrates knowledge of physical therapy procedures and equipment allowing for safety andmaximumpatient benefits.
- Attend all department related education activities andmaintainsknowledge base of all equipment and procedures.
- Inspects all equipment before use to grantee patient safety.
- Instructs patients adequately before each treatmentsession asto purpose and effects of treatment.
- Assistsin keeping track of inventory itemsrequiredfor smooth operation.
- Attends continuing education seminars tomaintainknowledge related to best practices.
- Performsappropriate documentationtomaintainthe departmental standard.
- Documents accurately, the progression and outcome following each patient treatment, along with the patient's reaction to session.
- Documents accurately the information necessary toinitiatecharges forabovetreatment sessions.
- Monitors circumstances surrounding patient treatment and documents occurrences which are not consistent with department policies/procedures.
- Participates in communication with other involved hospital personnel toprovide forquality patient care and cost-efficient programming.
- Communicates efficiently and effectively on an ongoing basis with colleagues and patients.
- Communicates to departmentadministrationany suggestions which may improve operations.
- Provides confident instruction to patients and family members in preparation for discharge or for carryover ofhome exercise programs.
- Performs other duties as outlined to improve the overall effectiveness of the department as both a treatment facility and an educational environment.
- Helpsmaintaina safe and orderly department.
- Strives to makegood useof time; seeks out work which needs to be completed.
- Demonstrates a complete understanding of hospital fire and safety procedures.
- Promotes a safe and efficient working environment by adhering to hospital and department policies and procedures.
- Adheres to and promotes Infection Control policies and procedures.
- Adheres to safety regulations reporting any variance to the Director of Rehabimmediately.
- Patient relations-Demonstratesregard for the dignity and respect of all patients, family members,visitorsand hospital personnel as defined in the philosophy of Health-Quest and Putnam HospitalCenter.
- Displays a caring and courteous attitude and respect for all patients, family members,visitorsand hospital personnel as defined by the Mission and Vison ofHealth-Quest.
- Promotes and contributes positively to intra-departmental and inter-departmental relationships.
- Adheres to department and hospital dress code wearing picture identification 100% of the time.
Education and Experience Requirements:
Graduated from an accredited physical therapy school with aB.S./M.S./DPT
degree.MinimumKnowledge,Skillsand AbilitiesRequirements:
