Job Description

Full time or Part time opportunity!
Position Summary:
The primary purpose of this position is to provide skilled physical therapy services according to recognized standards of practice. Perform comprehensive evaluations within twenty-four hours to referral. Plan and implement an appropriate treatment program with measurable, objective, realistic goals based upon evaluation data. Complete the following documents as necessary following evaluation: contracture evaluation, balance sheet and physician certification for restorative treatment and therapy attendance sheet. Consistently demonstrates safe resident care techniques. Maintain legible, complete records of residents' evaluations, progress notes and discharge notes in the SOS computer system and the residents' charts. Work as an integral part of the rehabilitation team to meet resident needs. This includes working within the departmental team as well as resident/family instruction, physician/nurse practitioner contact, and contact with involved health professionals. Perform resident treatments as outlined in evaluation with appropriate changes as residents' status changes. Treatments may include but are not limited to transfer training, bed mobility, ambulation, exercises, massage and use of electrical or batter-operated modalities. Attend scheduled family and clinical meetings. Assist with the supervision and training of new personnel, assistants, aides, students and volunteers. Attend continuing education courses/ seminars as appropriate with follow-up in-services to the department.
Qualifications:
Education & Experience:
Minimum BS degree in Physical Therapy. Geriatric rehabilitation experience preferred.
Pay Rate:
