Job Description

Physical Therapist Perdiem $47.00-$50.50/hr Everything we do at The Wesley Community is guided by our Core Values which form the acronym "CARES" . What will I do as a Physical Therapist for The Wesley Community? Our Physical Therapists work closely with in our in-patient long term care residents and short term rehab patients to restore activities of daily living. A few of the most common duties and responsibilities required of our Physical Therapists include: Evaluate and develop treatment plans for residents based on their medical diagnoses. Implement treatment plans to improve residents' goals toward functional independence. Assess and document residents' progress in detailed re-certifications, progress reports, and daily notes. Maintain resident confidence by ensuring the confidentiality of all information. Maintain documentation of resident care services by auditing resident and department records. Assume responsibility for clinically supervising occupational therapy assistants. Participate in care plan meetings and contribute to related documentation. Plan, develop, and organize PT-related reports and data tracking as directed by the Rehab Director. Assist in departmental projects as needed.
Physical Therapist Requirements :
Graduate of an accredited Physical Therapy Program. Current New York State Physical Therapist License. Experience in Subacute and/or Long-Term Care is a plus, though not required.
