Job Description:
As a PT in a Skilled Nursing Facility environment, you will be evaluating residents, developing treatment plans, and obtaining approval for service from the referring physician. Collaborate with an experienced team of professionals that are committed to helping each patient recover their strengths, skills, independence, and confidence.

PT Responsibilities:
Provide individual treatment plans for patients to restore muscular and neurological function to maintain a good quality of life Assess effectiveness of treatment program and modify goals/treatment methods, as need to meet changing needs Evaluate and document treatment/progression of each patient in compliance with department standards Provide updates on progress and training for patients, caregivers, families, as well as other health team personnel on using assistive modalities, and appropriate physical therapy procedures Perform all other duties as assigned and as required to effectively discharge the responsibilities of the position
Requirements:
NY Physical Therapy License Experience implementing and coordinating PT treatment plans Strong interpersonal and communication skills
