Our mission is to help patients improve mobility, restore function, prevent future injuries, and maintain independence through personalized treatment and education. Unlike traditional home health agencies, we do not perform OASIS assessments. Our therapists are able to focus on what they do best - delivering exceptional patient care without the administrative burden commonly associated with home health. White Plains Hartsdale Yonkers Eastchester Scarsdale Surrounding areas Responsibilities Evaluate and treat patients in their homes under Medicare Part B Develop and implement individualized treatment plans based on patient needs and goals Provide hands-on therapy, therapeutic exercise, balance training, and patient education Communicate with the care coordinator regarding referrals and scheduling Maintain timely, accurate, and compliant clinical documentation Collaborate with physicians and other healthcare providers as needed Promote patient independence, safety, and long-term wellness Qualifications Doctor of Physical Therapy (DPT) degree from an accredited program Active New York State Physical Therapy license Current CPR certification Ability to work independently and manage a caseload efficiently Reliable transportation and willingness to travel to patient homes Strong interpersonal and communication skills Excellent clinical reasoning and problem-solving abilities Proficiency with computers and electronic documentation systems Compassionate, professional, and patient-centered approach to care New graduates are encouraged to apply.
