Develop and execute customized treatment plans utilizing evidence-based physiotherapy techniques such as trigger point therapy and orthopedics. Assist patients in managing acute care needs, geriatrics, pediatrics, or post-surgical rehabilitation through targeted interventions. Educate patients and their families on exercises, injury prevention strategies, and home care routines to support ongoing recovery. Collaborate with interdisciplinary teams to coordinate discharge planning and ensure seamless transitions across care settings. Maintain accurate documentation of patient progress, treatment sessions, and care plans within electronic medical record (EMR) systems. Stay current with advances in physiology knowledge, anatomy, medical terminology, and physiotherapy practices to deliver optimal care. Requirements Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction. Proven experience in inpatient hospital settings, outpatient clinics, or home & community care environments. Strong knowledge of physiology, anatomy, medical terminology, and specialized techniques such as trigger point therapy. Demonstrated ability to develop effective care plans for diverse patient populations including geriatrics and pediatrics. Familiarity with EMR systems for documentation and patient management. Excellent communication skills to educate patients and collaborate with healthcare teams effectively. Experience in discharge planning and managing care across multiple settings such as acute care, home health, or outpatient clinics is highly desirable.
