Geriatric Physical Therapist

Job Summary The Home Health Physical Therapist (PT) provides skilled physical therapy services to patients in their homes, focusing on improving mobility, restoring function, and promoting independence. This role involves working independently while coordinating care with physicians and an interdisciplinary team, in compliance with Ohio state practice regulations and home health standards. Key Responsibilities Perform initial and ongoing patient assessments in the home setting Develop and implement individualized plans of care based on patient needs and physician orders Deliver hands-on therapy, therapeutic exercise programs, gait training, balance training, and fall prevention strategies Evaluate home environments for safety and recommend modifications or adaptive equipment Educate patients and caregivers on treatment plans, home exercise programs, and disease management Accurately document visits, progress, and outcomes in compliance with Medicare and agency guidelines Communicate effectively with physicians, nurses, and other members of the care team Participate in case conferences and care coordination activities Manage a flexible daily schedule and travel within assigned territory Qualifications Doctor of Physical Therapy (DPT) or equivalent from an accredited program Active Physical Therapist license in Ohio Minimum 1 year clinical experience in home health, PREFERRED Valid driver's license, reliable transportation, and auto insurance Ability to work independently and manage time effectively Strong communication and documentation skills Work Environment Patient homes across assigned geographic area Frequent travel required Variable conditions depending on patient home environments Compensation & Benefits Competitive hourly wages with mileage reimbursement Health, dental, and 401K (for eligible employees) Paid time off and holidays (for eligible employees) Flexible scheduling

Pay:

$50.00 - $60.00 per hour
