Job Description

PRN Physical Therapist - Home Health | Flexible Schedule Lorain County / Western Cuyahoga / Medina / Erie County, OH A growing home health organization is seeking compassionate and motivated Physical Therapists for PRN / part-time opportunities throughout Northeast Ohio. This is a great opportunity for therapists looking for:

  • Minimal travel between visits Compensation
  • $50-$62 per hour
  • Higher pay available for evaluation visits
  • Mileage reimbursement available
  • Paid orientation and EMR training Schedule Flexibility Create your own schedule based on your availability.
Examples:
  • 1-2 days per week
  • Afternoons or evenings only
  • Supplemental PRN work
  • Flexible weekly caseloads The organization is open to therapists who want limited or expanded schedules. Patient Population Primary patient population includes geriatric patients in:
  • Home health settings
  • Rehabilitation centers
  • Skilled nursing / partner facilities Common cases include:
  • Orthopedic conditions
  • Post-acute recovery
  • Balance & fall prevention
  • Post-surgical rehabilitation Responsibilities
  • Evaluate and treat patients in home/facility settings
  • Develop individualized care plans
  • Complete timely documentation using KanTime EMR
  • Educate patients and caregivers
  • Collaborate with healthcare teams to ensure quality patient care Qualifications
  • Active Ohio Physical Therapist license
  • Valid driver's license and reliable transportation
  • Strong communication and organizational skills
  • Home health experience preferred, but not required
