Celina Health Care Center in Celina, OH is seeking a PRN Physical Therapist (PT) to join our growing In-House therapy team . Essential Duties & Responsibilities:
Evaluate residents' physical conditions and develop individualized treatment plans based on physician orders and clinical needs. Provide therapeutic interventions focused on improving strength, mobility, gait, balance, endurance, and pain management. Establish realistic functional goals and modify treatment plans as needed based on progress. Complete accurate and timely documentation in compliance with Medicare, Medicaid, and facility guidelines. Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and families regarding resident progress and discharge planning. Educate residents, families, and caregivers on treatment plans, exercises, and safety precautions. Participate in care plan meetings and interdisciplinary team discussions. Ensure compliance with all state, federal, and company policies, including infection control and safety standards. Supervise and direct Physical Therapist Assistants (PTAs) and rehabilitation aides as appropriate. Support facility initiatives related to quality measures, fall prevention, and restorative nursing programs.
Qualifications:
Graduate of an accredited Physical Therapy program. Current state licensure as a Physical Therapist. Previous experience in skilled nursing, long-term care, or geriatric rehabilitation preferred. Strong knowledge of Medicare Part A, Part B, and managed care therapy guidelines. Excellent communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Strong clinical assessment and documentation skills. All inquiries will be held confidential
