What You'll Do:

Evaluate and treat residents in a skilled nursing/long-term care environment Develop and implement individualized treatment plans Collaborate with PTAs, OTs, COTAs, SLPs, and nursing staff Maintain accurate and timely documentation using Net Health and PCC Ensure compliance with all regulatory and clinical standards

What We're Looking For:

Licensed Physical Therapist in the state of Ohio Experience in SNF/LTC preferred, but not required Strong communication and teamwork skills Passion for delivering high-quality, compassionate care
