In this position, you will provide skilled home health physical therapy services to adult and geriatric patients in Lancaster and surrounding areas. Typical responsibilities include completing comprehensive in-home evaluations, developing and implementing individualized treatment plans, and collaborating closely with nurses, occupational therapists, speech therapists, and other members of the care team. You will focus on restoring function, improving mobility, reducing fall risk, and helping patients safely remain in their homes following illness, injury, surgery, or hospitalization. Your day will involve traveling between patient homes, performing 57 visits per day depending on caseload and driving distances, and documenting care in an electronic medical record system. You will provide hands-on interventions such as therapeutic exercise, balance and gait training, neuromuscular re-education, and caregiver training, along with education on home exercise programs and safety in the home environment. The position typically offers flexible scheduling within daytime hours, allowing for a degree of autonomy and control over your daily visit routes, while still maintaining clear expectations for productivity and documentation timeliness. You will work closely with a supportive clinical leadership team that values collaboration, open communication, and high-quality patient outcomes.
