InCare Ohio Home Health is looking for a part time Licensed Physical Therapy Assistant. Flexible schedule. This will be for Mahoning and Trumball Counties. Job Summary Join our dynamic healthcare team as a Physical Therapist Assistant (PTA) specializing in Home Health! In this vital role, you will deliver personalized therapeutic care directly in patients' homes, empowering individuals to regain mobility, reduce pain, and enhance their quality of life. Your expertise will support discharge planning, outpatient therapy, and acute care needs, ensuring seamless continuity of care across diverse settings. Responsibilities Provide skilled physical therapy interventions to patients in their homes, including exercises, manual therapy, and modalities such as trigger point therapy. Collaborate closely with licensed therapists, physicians, and other healthcare professionals to coordinate care and ensure smooth discharge planning. Educate patients and their families on exercises, safety precautions, and health maintenance techniques to promote independence. Document all patient interactions accurately in medical records, adhering to healthcare regulations and confidentiality standards. Support outpatient clinics or skilled nursing facilities by providing specialized physiotherapy services for orthopedics, geriatrics, pediatrics, and other populations. Skills Proven experience in home health settings with a strong understanding of inpatient and outpatient care dynamics. Knowledge of physiology, anatomy, and medical terminology essential for accurate assessment and treatment. Skilled in various physiotherapy techniques including trigger point therapy and manual therapy modalities. Familiarity with discharge planning processes and acute care protocols to facilitate seamless patient transitions. Experience working with diverse populations such as geriatrics, pediatrics, orthopedics, and those in skilled nursing facilities or clinics. Ability to develop comprehensive care plans aligned with patient goals and clinical best practices.
